1. Sponsor
The RevEHR Travel Giveaway (the “Sweepstakes”) is sponsored by:

2. Eligibility
The Sweepstakes is open only to legal US residents who are:
18 years of age or older, and
Currently employed by an active optometric practice in the United States
Employees of Sponsor, its affiliates, subsidiaries, advertising and promotion agencies, and immediate family members or persons living in the same household are not eligible to enter or win.
Void where prohibited by law.
3. Sweepstakes Period
The Sweepstakes begins on June 22, 2026 at 12:01 AM EDT and ends on August 31, 2026 at 11:59 PM EDT (the “Sweepstakes Period”).
4. How to Enter
Eligible participants may enter during the Sweepstakes Period by:
Booking a RevolutionEHR product demo
Alternate Method of Entry (AMOE)
Eligible participants may submit a mail-in entry by hand-printing their full name, practice name, email address, and ChiroTouch account number on a 3” x 5” card and mailing it to:

Mail-in entries must be postmarked during the Sweepstakes Period and received no later than September 1, 2026.
Limit one (1) entry per person regardless of entry method.
By participating in the Sweepstakes during the Sweepstakes Period, entrants agree to be bound by these Official Rules.
5. Prize
One (1) Grand Prize will be awarded:
A vacation package with an approximate retail valued (ARV) of $5,000. Prizes are non-transferable and cannot be exchanged for cash and are subject to all applicable terms and conditions.
Winner is solely responsible for all federal, state, and local taxes associated with the Grand Prize. Sponsor may issue an IRS Form 1099 as required by law.
Sponsor makes no warranties or representations regarding the Grand Prize and disclaims any implied warranty.
6. Winner Selection
One (1) potential winner will be selected in a random drawing from all eligible entries received during the Sweepstakes Period.
The drawing will occur on or about September 1st, 2026.
Odds of winning depend on the total number of eligible entries received.
7. Winner Notification & Verification
The potential winner will be notified via the email address provided as part of their entry method within approximately five (5) business days of the drawing.
To claim the prize, the winner must:
Complete and return an Affidavit of Eligibility and Liability/Publicity Release, which Sponsor will provide;
Submit a completed IRS Form W-9 for tax purposes; and
Provide any additional documentation reasonably requested by Sponsor
Failure to respond within ten (10) business days may result in disqualification and selection of an alternate winner.
If a potential winner is disqualified, declines the Grand Prize, or fails to timely respond, Sponsor may select an alternate winner from the remaining eligible entries.
8. Publicity
Except where prohibited by law, acceptance of the Grand Prize constitutes permission for Sponsor to use the winner’s name, likeness, biographical information, and testimonial for advertising, marketing, publicity, and promotional purposes without additional compensation.
9. General Conditions
Sponsor reserves the right to cancel, suspend, or modify the Sweepstakes at any time.
Sponsor reserves the right to disqualify any individual who tampers with the entry process or violates these Official Rules.
10. Limitation of Liability
By participating, entrants agree to release and hold harmless Sponsor and its affiliates, subsidiaries, officers, directors, employees, and agents from and against any claim or cause of action arising out of participation in the Sweepstakes or receipt, use, or misuse of any prize.
11. Privacy
Information collected in connection with this Sweepstakes is subject to Sponsor’s Privacy Policy available at: https://www.revolutionehr.com/privacy-policy
12. Governing Law
The Sweepstakes and these Official Rules are governed by the laws of the State of California, without regard to conflict of law principles.
Any disputes arising out of or relating to the Sweepstakes shall be resolved exclusively in the federal or state courts located in San Diego County, California.
13. No Purchase Necessary
NO PURCHASE OR PAYMENT OF ANY KIND IS NECESSARY TO ENTER OR WIN.
A PURCHASE WILL NOT INCREASE YOUR CHANCES OF WINNING.
